Apart from the unbelievable From address, it is easy to see how they used the first part of the recipient's e-mail address in the greeting. For example if your e-mail address is john.doe@example.com, the greeting would have read: Good afternoon!, john.doe
Everyone can see this was done by a machine, simply copying the first part of your e-mail address and not really addressing you by name.
Right, they already greeted you and in the very next sentence they greet you again. Anyway, how do you delivery the post package? The poor English and strange punctuation already tells you that this e-mail did not come from the UPS.
This sentence also sounds like a machine speaking to you.
All the links in this e-mail pointed to the actual UPS website, to add a false sense of security and to make the e-mail look more legitimate. The only thing that's dangerous in this e-mail is the attachment and that is the only thing the scammer wants you to open, because that will download the malware directly to your system.
So do not open the attachment, because there will be no form to fill out, only malware waiting to infiltrate your computer.
